Please indicate your role or relationship (item 39) by letting us know if you are an elected official (school board or city council person), a staff member of a municipality/school district, or a member of the general public. Item 39, your role, is required, but all other personal information and location items are optional.

You will need to respond to all ‘required’ items and any validation errors before your survey responses are recorded. You will be notified of any errors after hitting the Submit Survey button. You can then go to the items needing attention, enter your response and Submit Survey again, repeating until all errors are resolved. Please resolve all errors before you Submit Survey again.
