Identify your Role (question 39) by letting us know if you are an ‘Elected Official’, a government ‘Staff’ member, or a member of the general ‘Public’.

Your Role and the Survey Code are required, but all other personal and address/email information is optional.

You will be notified of any errors after hitting the ‘Submit Survey’ button. You must then return to the items needing attention, enter a response, then ‘Submit Survey’ again, until all errors are resolved. You will be shown an ‘End of Survey – THANK YOU’ page on successful submission.
